Title: Physical Therapist
Department: Rehabilitation
Reports to: Director of Rehabilitation
A. Essential Functions
-
Skills and expertise in current evaluation and remediation techniques.

Evaluates patients, develops effective treatment plans and obtains orders from the referring physician.

Communicates well with supervisor and other health team members regarding patient progress, problems, and plans.
-
Records evaluations, daily treatment notes, weekly progress notes, and discharge summaries per policy and procedure.
-
Complies with the Physical Therapy Act in the State.
-
Supervises Physical Therapy Assistants and/or Physical Therapy Aides and RNAs in direct resident care and patient related activities.
-
Flexible hours as agreed upon by the Director of Rehabilitation and direct supervisor.
B. Major Duties and Responsibilities
-
Participates in patient care conferences and Rehabilitation Conferences.
-
Participates in in-service training program for other staff in the facility.
-
Records treatment charges per policy and procedure.
-
Instructs patient’s families or nursing staff in maintenance program.
-
Participates in discharge planning.
-
Secure necessary durable medical equipment for residents to facilitate independence in mobility (i.e. braces, walkers, canes, elevated toilet seats, and grab bars).
-
Assists with monthly wheelchair clinic.
-
Reports any problem with department equipment so that it is maintained in good working order.
-
Acts as a clinical preceptor for affiliating Physical Therapy or Therapy Assistant Schools.
-
Performs other duties as assigned.
