Title: Physical Therapist 

Department: Rehabilitation 

Reports to: Director of Rehabilitation 

 

A.  Essential Functions   

  • Skills and expertise in current evaluation and remediation techniques.  

  • Evaluates patients, develops effective treatment plans and obtains orders from the referring physician.  

  • Communicates well with supervisor and other health team members regarding patient progress, problems, and plans.  

  • Records evaluations, daily treatment notes, weekly progress notes, and discharge summaries per policy and procedure.  

  • Complies with the Physical Therapy Act in the State.  

  • Supervises Physical Therapy Assistants and/or Physical Therapy Aides and RNAs in direct resident care and patient related activities.   

  • Flexible hours as agreed upon by the Director of Rehabilitation and direct supervisor. 

 

B. Major Duties and Responsibilities 

  • Participates in patient care conferences and Rehabilitation Conferences.  

  • Participates in in-service training program for other staff in the facility.  

  • Records treatment charges per policy and procedure.  

  • Instructs patient’s families or nursing staff in maintenance program.  

  • Participates in discharge planning.  

  • Secure necessary durable medical equipment for residents to facilitate independence in mobility (i.e. braces, walkers, canes, elevated toilet seats, and grab bars).  

  • Assists with monthly wheelchair clinic. 

  • Reports any problem with department equipment so that it is maintained in good working order.  

  • Acts as a clinical preceptor for affiliating Physical Therapy or Therapy Assistant Schools.  

  • Performs other duties as assigned.

Salary : $50 - $54
