Position Summary: The Case Planner works collaboratively with therapists, education specialists, and social workers to manage a caseload of approximately 10 children with varying mental health needs. The role focuses on providing intensive casework and field services to support permanency goals, while also conducting outreach, coordinating Family Team Conferences, maintaining case records, utilizing the CONNECTIONS system, and participating in case consultations.
Essential Duties
Children’s Aid will conduct a prompt and thorough investigation of all allegations of discrimination, harassment, retaliation, or any violation of the Equal Employment Opportunity Policy in a confidential manner and will take appropriate corrective action, if and where warranted. Children’s Aid prohibits retaliation against employees who provide information about, complain about, or assist in the investigation of any complaint of discrimination or violation of the Equal Employment Opportunity Policy.
The incumbent is expected to work a schedule, whether in-person or hybrid, as determined by the department's needs to facilitate effective collaboration with the team. This flexibility is essential for maintaining seamless communication, fostering teamwork, and ensuring the smooth operation of the department's processes. The position's requirements are subject to periodic review and adjustment based on organizational needs and changes in work dynamics.

Essential Duties
- Understand and promote the organization’s Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
- Work with a dynamic team of professionals including therapists, education specialists, and social workers in our foster care program for children.
- Manage a case load of approximately 10 children with varied levels of mental health needs.
- Provide intensive casework services and fieldwork services for children and their birth and foster families with the goal of providing permanency as quickly as possible.
- Conduct outreach to connect children and families with agency and community treatment resources.
- Utilization of CONNECTIONS, child welfare computer system.
- Coordinate Family Team Conferences.
- Actively participate in case consultations
- Maintain written case records.
- Perform other duties as assigned.
- Bachelor’s Degree required.
- Foster Care experience preferred.
- Bilingual preferred.
- Valid Driver’s License.
- Excellent verbal, writing, and organizational skills.
- Flexibility, and ability to work under pressure and meet deadlines.
- Computer literacy.
