Salary: $66,300 - $70,000/yr

Position Summary: Family Stabilization (FS) Senior Case Planners are responsible for providing culturally competent, short-term Trauma Focused-Cognitive Behavioral Treatment (TF-CBT) to families seeking Family Stabilization services in the boroughs of the Bronx, Manhattan, Brooklyn and Staten Island.

The Family Stabilization Program provide short term, intensive, home and community-based therapeutic prevention services to adolescents and their families who have sought FAP/PINS intervention to address the youth's problematic behaviors. The Family Stabilization Program has two service tracts. A Crisis Intervention tract for families and children who face an episodic crisis and a Trauma Focus tract for families and children that have been subjected to traumatic experiences such as acts of violence like rape or assault experienced in the home and/or community, etc. The trauma-focused tract utilizes the TF-CBT model which is an evidenced based treatment model that integrates trauma-sensitive interventions with cognitive-behavioral strategies. The goals of TF-CBT is to reduce children's negative behavioral, physical, emotional and cognitive responses to trauma; correct maladaptive or unhelpful beliefs related to the traumatic experience.

Responsibilities: General responsibilities for the position of Senior Case Planner (FS) include, but are not limited to:

  • Understand and promote the organization's Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
  • Carry caseload of approximately 10 FS cases.
  • Attend all TF-CBT trainings and receive certification in the model as required; as well as all mandated ACS trainings.
  • Participate in all weekly Staffing with TF-CBT trainer/consultant.
  • Ensure timely completion of ACS ADVPO Progress Notes or Connections Progress Notes.
  • Implement all TF-CBT protocols and interventions as provided by the model consultant and/or FS supervisor.
  • Administer the TF-CBT treatment consistent with the model to ensure fidelity and adherence to the model.
  • Fulfill all OCFS, and ACS accountability requirements including timely completion of FASPs, Progress Notes and Plan Amendments
  • Perform system updates in PROMIS, CONNX and eVOLV.
  • Complete all home-based contacts and seek to maintain four case work contacts per month per family as specified by the model.
  • Engage in outreach activities in the community in order to attain required 20% utilization from self-referrals as required by contract.
  • Be available by phone 24/7 or as needed to respond to client crises. This includes ability to conduct sessions on weekends as needed, to accommodate client availability.
  • May be required to accept cases in other boroughs where we have the FS contracts (Bronx, Brooklyn, Manhattan, Staten Island.
  • Collaborate with Educational and Employment specialist and/or Case Aide to monitor school attendance, obtain school records and intervene with DOE when appropriate to develop an appropriate education/vocational plan for youth.
  • Ability to work flexible hour, including working remotely in the field, community or various CA offices to meet the needs of families and improve efficiency and management of time and resources.
  • Perform other duties as assigned.

Minimum Qualifications

  • Master's Degree in Social Work (MSW) or equivalent degree in any NYS license-eligible Mental Health profession required.
  • Bilingual English/Spanish is preferred but not required.

  • Ability and willingness to work with families and teens in the home, school and other appropriate settings in the community.

  • Ability to work independently, as well as contribute to a team.
  • Strong clinical skills and ability to work under pressure with high risk, crisis-oriented cases.
  • Very good interpersonal/communication skills, including ability to write well and meet copious amount of documentation in a timely manner.
  • Computer literate.
