We are seeking a passionate and compassionate Advocate – Stabilization Phase to join our team. In this role, you will provide intensive advocacy and case management services to women, children and families who require ongoing support beyond their initial engagement with the CFAC. You will help families navigate complex challenges, reduce barriers to safety and stability, connect with critical resources, and coordinate services alongside our multidisciplinary partners. This role also provides advocacy and support for families participating in our residential program while helping them build long-term stability and self-sufficiency.

Responsibilities could include:

Direct Support: Provide trauma-informed advocacy, emotional support, crisis intervention, and practical problem-solving for children and families. Build trusting relationships while helping families overcome barriers and remain engaged in services.

Case Management: Develop individualized stabilization plans that address safety, housing, transportation, healthcare, counseling, financial needs, and other family goals. Monitor progress and adjust plans as needs evolve.

Residential Advocacy: Support families participating in residential services by assisting with goal planning, program engagement, transition planning, and coordination of services that promote long-term family stability.

Resource Coordination & Advocacy: Connect families with internal programs and community resources while serving as a liaison with medical providers, counselors, schools, law enforcement, prosecutors, and other multidisciplinary team partners to ensure coordinated care.

Documentation: Maintain timely, accurate, and confidential documentation of client contacts, stabilization plans, referrals, service coordination, progress toward goals, and required outcome data.

Collaboration & On-Call Support: Participate in multidisciplinary team meetings, case reviews, and collaborative planning. Provide services during regular office hours and assigned on-call rotations, responding to crisis situations and helping initiate services for newly referred families.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Human Services, Criminal Justice, or a related field. A Master's degree is a plus.
  • Experience working with children and families in child advocacy, victim services, social services, behavioral health, domestic violence, or a related field preferred.
  • Knowledge of trauma-informed care, crisis intervention, child development, family systems, and community resources.
  • Strong communication, organizational, and relationship-building skills with the ability to collaborate effectively across multidisciplinary teams.
  • Ability to manage a caseload, prioritize competing responsibilities, and respond effectively in crisis situations.
  • Strong documentation skills with a commitment to confidentiality, professionalism, and ethical practice.
  • Fluency in Spanish is a plus.
