Job Title & Specialty Area: Community Health Worker

Department: Social Work (Emergency Department)

Location: Children's Health- Dallas

Shift: Full-Time; a rotating Mid-shift during the weekday from 11:00a to 7:30p and rotating working every other weekend Saturday and Sunday from 9:30a to 6:00p. (For example, Week 1: will be expected to work Monday through Friday from 11:00a to 7:30p and Week 2: will work be expected to work Sunday 9:30a- 6:00pm, Wednesday, Thursday, Friday from 11:00a-7:30p and Saturday from 9:30a- 6:00p.

Summary:
The Community Health Worker (CHW) serves as a trusted liaison between community members and healthcare providers, building strong relationships to improve access to care, increase health literacy, and support overall well-being. CHWs educate individuals on insurance coverage options, help establish connections with primary care and/or specialty care providers, and guide appropriate use of healthcare services. They address safety concerns and connect individuals with community resources to meet basic needs. Through culturally responsive education, outreach, and care coordination, CHWs play a vital role in supporting underserved populations, addressing social determinants of health, reducing disparities, and improving health outcomes.

Responsibilities:
  • Administer screening tools to assess barriers to accessing primary care or other needed services. Review common challenges contributing to limited use of preventive care and conduct follow-up to determine whether connections to a primary care provider or other resources have been established. Address child safety topics, such as car seats and immunizations, as appropriate.
  • The following responsibilities may vary depending on the CHW’s function and department assignment:
  • Organize community events, collect health data, advocate for client needs and assist clients in navigating healthcare and social service systems (Home visit or virtual requirements are based on CHW specific role).
  • Evaluate barriers preventing patients and families from establishing or utilizing a primary care provider (PCP), and provide guidance and resources to support connection with appropriate community healthcare providers.
  • Deliver culturally appropriate health education on topics such as chronic disease prevention, nutrition, physical activity, and medication adherence.
  • Facilitate workshops, support groups, or educational sessions in community.
  • Distribute educational materials and resources tailored to community needs.
  • Document all interactions, referrals, and outcomes in compliance with organizational standards.
  • Collaborate with healthcare providers, social workers, and community organizations to improve service delivery.
  • Identify barriers to care and connect clients with appropriate resources such as food assistance, housing support, transportation, and mental health services.
  • Participate in team meetings, training sessions, and continuing education to enhance skills and stay updated on community resources.
  • Maintain and build existing relationships with community leaders, community-based organizations, churches, schools, daycares, etc.
  • Attend events in the community to include schools, churches and health fair events.


How You’ll Be Successful:

WORK EXPERIENCE
  • At least 1 year job related experience Required
  • At least 2 years job related experience Preferred

EDUCATION
  • High school diploma or equivalent Required

LICENSES AND CERTIFICATIONS
  • Community Health Worker License 180 Days Required
