CADES seeks a Healthcare Supervisor to further our mission of empowering those with special needs to achieve their highest potential.
Position Description: The Healthcare Supervisor (HS) provides clinical care and case management to approximately 15 individuals with medical fragility, Intellectual and Developmental Disabilities (IDD) and Mental Health (MH) diagnoses. The individuals served reside in group homes regulated by the state of Pennsylvania under the 6400 regulations. The HS utilizes clinical and case management skills to ensure individuals receive appropriate, high-quality care.
Essential Duties and Responsibilities:
- Coach and train staff on skills such as, but not limited to, how to give bed baths, repositioning and taking vitals; identify, advise, and train staff on illnesses and signs to look for that may indicate an imminent emergency.
- Communicate and collaborate with external therapists and other related services providers such as contracted nurses, occupational therapists (OT), physical therapists (PT), and Speech (SLP) to ensure continuity of care.
- As an expert, train others in the proper use of all prescribed adaptive equipment for individuals .
- Act as the primary contact for external physicians and specialists.
- Utilize the established pharmacy system to order new medications or refills.
- Support staff in medication administration record (MAR) documentation per 6400 regulations.
- Coordinate care during ER visits and hospitalizations.
- Keep electronic and paper charts updated with the most current information so staff may have correct information regarding each individual to ensure their health and safety.
- Participate in rotating on-call duties as assigned
- Perform other duties as assigned
Qualifications:
- High school diploma or graduate equivalency diploma (GED) and completion of practical nursing program with active, valid licensure as a Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania
- Demonstrated experience working with feeding tubes, colostomies, tracheotomies, catheter, and wound care strongly preferred
- Two (2) years’ experience working with individuals living with multiple disabilities, including a strong understanding of the safety and communication support required for individuals with physical disabilities
- Prior experience with Community Living Arrangements preferred
- Demonstrated ability to work in a fast-paced environment and effectively prioritize tasks
- Demonstrated strong attention to detail
- Demonstrated ability to work effectively as part of a team
- Demonstrated strong time management and organizational skills
- Demonstrated excellent judgment with the ability to independently solve problem and make decisions with little direct supervision
- Demonstrated ability to anticipate changes and/or identify issues and take appropriate and effective actions
- Ability to work nights and weekends as needed
- Other duties as assigned
This position requires the ability to perform the essential functions of the job, including the ability to lift a minimum of 20 pounds, bend, twist, and turn.
