We provide trauma-informed care, a safe and structured environment, and individualized programming to support healing, growth, and reintegration.

We are currently seeking a compassionate, organized, and mission-driven Intake/Discharge Coordinator to serve as a critical point of contact for youth entering and exiting our care.

Position Summary

The Intake/Discharge Coordinator plays a vital role in ensuring a smooth, respectful, and efficient transition for youth entering and leaving our program. This position is responsible for overseeing all intake and discharge processes, including assessments, documentation, coordination with external agencies, and family engagement. The ideal candidate is detail-oriented, trauma-informed, and able to work collaboratively with youth, families, and interdisciplinary team members.

Key Responsibilities

Intake Process

  • Serve as the first point of contact for new youth referrals
  • Schedule and conduct intake interviews and assessments with youth and their families
  • Collect, review, and maintain all required admission documentation
  • Collaborate with case managers, clinicians, and medical staff to determine readiness and placement needs
  • Ensure the youth’s transition into the program is trauma-sensitive, welcoming, and well-coordinated
  • Communicate clearly with placing agencies, legal guardians, and other stakeholders

Discharge Planning

  • Coordinate discharge planning meetings with the treatment team, family, and external partners
  • Ensure all documentation and discharge summaries are completed accurately and on time
  • Facilitate a smooth transition to home, another placement, or step-down services
  • Provide referrals and connections to aftercare services when appropriate
  • Follow up post-discharge when required by program standards

Documentation & Compliance

  • Maintain up-to-date and confidential client records in accordance with state regulations and organizational policies
  • Track and report on admissions, discharges, and lengths of stay
  • Ensure compliance with licensing, reporting, and auditing requirements

Qualifications

  • Associate’s or Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Bachelor’s preferred)
  • 2 years of experience working with at-risk or system-involved youth
  • Familiarity with trauma-informed care, juvenile justice, foster care, or residential treatment preferred
  • Strong organizational and communication skills
  • Ability to work under pressure and manage multiple priorities
  • Experience with documentation and client management systems
  • Must pass background checks and possess a valid driver’s license
