Health and Medical Liasion

Job Summary:

TheHealth and Medical Liasion supports the daily operations of services provided to individuals living in group homes. This role is primarily responsible for transporting youths to appointments. The ideal candidate is highly organized, detail-oriented, prompt, and comfortable working in a team environment focused on supporting individuals with developmental or behavioral health needs.

Job Type: Full-time

Schedule: Monday - Friday: 8:30 AM - 4:30 PM

Pay: $22 per hour

Location: This position covers Lehigh, Carbon and Wayne Counties

Key Responsibilities:

• Transport youths to essential appointments with company vehicle.
• Coordinate transportation arrangements as needed.
• Maintain accurate and up-to-date appointment logs and calendars.
• Assist program staff with the completion of forms, reports, and internal communications.
• Collaborate with case managers, direct support professionals, and health professionals to ensure continuity of care.
• Serve as a liaison between group home staff, residents, and outside providers to facilitate seamless communication and service delivery.
• Advocate for the timely scheduling and completion of necessary services for residents.

Qualifications:

• High school diploma or equivalent required; associate’s degree in human services, healthcare administration, or related field preferred.
• At least one year of experience in an administrative or support role, preferably in a healthcare, group home, or social services setting.
• Strong organizational and time-management skills.
• Ability to manage multiple priorities and meet deadlines.
• Excellent communication and interpersonal skills.
• Proficient in Microsoft Office Suite (Word, Excel, Outlook).
• Knowledge of HIPAA and confidentiality standards.
• Ability to work independently and as part of a team.
