Summary
The Family Support Navigator will provide our clients with consistent, high-quality care through an array of evidence-based, culturally competent, person and family-centered treatment. The Family Support Navigator will assist in providing behavioral health services, including access to same-day services. The Family Support Navigator will act as a liaison to ensure youth and families are connected to the appropriate behavioral health services including school based, peer support, and outpatient services.
SCHEDULE: Monday- 9a-530p, Tuesday- 8a-430p, Wednesday- 11a-8p, Thursday- 8a-430p, Friday- 12p-8p

Our goal is to support, reward and compensate the entire individual. Depending on role eligibility, your offer may include a bonus or other incentives. Therefore, final offer amounts may vary from the amount stated.

Job Responsibilities & Essential Functions
- Provide a safe, welcoming, and clean environment for our clients and families
- Provide trauma-informed, recovery-oriented, person-centered care based on the unique needs of clients
- Be knowledgeable of our Triage, Screening & Intake process with clients to cover this process when needed
- Work directly with the school-based director in managing the referrals for school-based counseling and triage any referrals not deemed appropriate for school-based.
- Assist with the facilitation of our family partner and peer support services, including individual, family, or group services as appropriate
- Provide clients and families with advocacy, coaching, training, support, and psychoeducation
- Support transitions of care and engage in effective coordination of care across health and behavioral health services to facilitate wellness and recovery of the whole person, including outreach and engagement, and after-care planning
- Utilize evidence-based interventions, including motivational interviewing skills to foster empowerment and develop person-centered service plans as appropriate
- Maintain record-keeping as required by CFS, funding sources, and accreditation authorities
- Participate in all supervision and training activities as required by CFS, funding sources, and accreditation authorities
