Position Summary

The Intensive Care Coordination (ICC) Supervisor will provide direct support and supervision to assigned staff within the ICC program. The ICC Supervisor is responsible for ensuring that the daily operations of the program are being provided with quality and that the program is meeting all contractual obligations with Montgomery County OCY, as well as agency policies and procedures. The ICC Supervisor will ensure that ICC staff provide individualized support and planning in a manner that is collaborative, person-centered, youth and family driven, strengths-based, transparent, culturally responsive, and proactive. The ICC supervisor will be a liaison with OCY to review referrals, address barriers to collaboration, and to ensure quality services and supports are being delivered.

Responsibilities

  • Collaborate with Montgomery County OCY staff and leadership as a liaison to ensure effective working relationship between CFF and OCY.
  • Be present onsite at OCY offices 1-2 days per week to build strong partnership and address barriers to progress proactively.
  • Provide direct supervision one hour per week at minimum to assigned ICC staff and document in EHR.
  • Attending ITM meetings as needed to ensure quality services and supports are being provided, and to address barriers as appropriate.
  • Ensure that ICC staff provide Intensive Care Coordination to assigned families that is collaborative, person-centered, youth and family driven, strengths-based, transparent, culturally responsive, and proactive.
  • Ensure ICC staff meet face to face with youth and family as needed, at home, school, shelter, detention center or the community and will contact family, treatment team, teachers, health care providers and natural supports, or via phone/email, telehealth as appropriate.
  • Ensure ICC staff act as the lead planning agent for assigned youth within the ICC program, providing leadership for the ITM planning process.
  • Actively collaborate with Montgomery County Children and Youth staff and leadership to develop a collaborative partnership in engaging assigned youth and families.
  • Ensure ICC staff prep, plan and debrief with all team members around the ITM meeting for assigned youth.
  • Document all efforts in agency EHR.
  • Prepare reports as requested to Director and OCY for each assigned youth.
  • Proactively address barriers to the planning process with Director, ICC team and OCY staff.
  • Celebrate successes with CFF and OCY team by acknowledging and highlighting above and beyond efforts of team members.
  • Support ICC staff as they actively plan for discharge from this level of care coordination and ensure appropriate level of care and support is in place with a warm hand off prior to discharge.
  • Receive training in various person-centered planning models as assigned.
  • Support Parent Per Support staff as they actively encourage a youth and family driven plan for discharge from this level of care coordination and ensure appropriate level of care and support is in place with a warm hand off prior to discharge.
  • Ensure that Parent Peer Support staff provide services to assigned families that are informed by their lived experience, collaborative, person-centered, family driven, strengths-based, transparent, culturally responsive, and proactive.
  • Support PPS staff as they actively encourage a youth and family driven plan for discharge from this level of care coordination and ensure appropriate level of care and support is in place with a warm hand off prior to discharge.
  • Perform additional duties or tasks, as assigned, or requested.

Required Qualifications

  • Master’s degree in social services or related field.
  • 1 or more years of experience with team dynamics and systems work, e.g. family systems.
  • 3 or more years of comprehensive child-serving systems experience.
  • 1 or more years of experience in program leadership.

Preferred Qualifications

  • Excellent case management, leadership, partnering and customer service skills.
  • Expertise with conflict resolution and management.
  • Receive training in High Fidelity Wraparound Care Coordination model.

Work Environment

  • Hybrid work environment both in a standard office environment, and remotely from home, as well as community settings to include client home, schools, court, doctor’s offices and hospitals.
  • This role requires weekly on-site presence, with one day per week in the Audubon office and one day per week in the Hatboro office.
