Salary Range (final salary offer is based on a number of factors, such as certifications, experience, and more):
$23.08 - $26/hour
PLUS
An additional $0.9615/hour Bilingual Spanish Differential
This is a field-based position that will travel throughout Los Angeles County.

MINIMUM QUALIFICATIONS:
- B.A. or B.S. degree in mental health or a related field with four years experience, or an A.A. degree with six years experience. Additional education may be substituted for job experience when appropriate.
- Demonstrated organizational skills and comfort, aptitude, and appropriate boundaries in dealing with the program’s clientele. Commitment to Mental Health Services Act principles, including strengths-based emphasis, field-based services, team approach, 24/7 coverage, and “whatever it takes” philosophy.
- Bilingual in Spanish required to meet the needs of the clients.
- Must maintain a valid California driver’s license, insurance and driving record which complies with agency’s insurance carrier standards.
- Must pass all applicable health, fingerprint/LiveScan and DMV clearances.
- Requires the ability to independently use a computer and learn necessary computer programs.
JOB PURPOSE:
The FSP Case Manager provides rehabilitation and case management services for clients consistent with their team treatment plan. Services will include assisting clients/families with improving, maintaining or restoring everyday functional/daily living skills. The FSP Case Manager also provides targeted case management services, including assessing for client’s case management needs. In addition, the Case Manager follows up with appropriate linkage, consults with team members, and provides referrals as needed.
ESSENTIAL JOB FUNCTIONS:
1. Facilitate client/family access to mental health programs/services (identify needs, assist with applications, reduce language barriers, etc).
2. Provide rehabilitation services to clients consistent with their treatment team plan.
3. Accompany and/or transport clients to activities inside the program or in the community as needed.
4. Provide support, training, information, and referral services.
5. Provide outreach and field-based services.
6. Facilitate linkage to housing.
7. Work as part of dedicated team.
8. Be on-call by phone 24/7.
9. Attend off-site resource fairs and similar events as assigned.
10. Attend all required meetings, supervision, and case reviews as assigned.
11. Meet agency service time and documentation expectations, while providing services in accordance with all programmatic and contractual requirements.
12. Maintains agency attendance standards for staff and department needs.
13. Performs all other duties as assigned.
WORK SCHEDULE:
FLSA Status: Non-Exempt – Full Time
Work-Week: Sunday – Saturday
- Actual work schedule as agreed with Direct Supervisor and based on business/program needs.
- Work Schedule subject to change based on Center’s needs.
