The CPLC Supportive Housing Services team is Empowering Lives through Housing Stability. The Housing Navigator EMPOWERS LIVES by serving individuals and families that experience multiple barriers to housing stability and connecting them to affordable housing opportunities and supportive service resources.

HOW WILL YOU MAKE AN IMPACT IN THIS ROLE?

  • You will empower residents to live independently and thrive by connecting elderly individuals, couples, and people with disabilities to vital community resources, supportive services, and educational opportunities.
  • You will strengthen each resident’s support network through an empowerment-based approach that promotes self‑advocacy, personal growth, and long‑term stability.
  • You will work collaboratively with property management and community partners, to assess resident needs, facilitate access to services, and foster a welcoming, supportive housing environment.
  • You will play a key role in advancing CPLC’s mission of Empowered Lives by enhancing residents’ quality of life, reducing social isolation, and promoting safe, inclusive communities where individuals can reach their highest level of self‑sufficiency.

SPECIALIZATIONS

1. Gerontology / Aging Services

  • Aging in place best practices
  • Cognitive and physical changes in older adults
  • Senior benefits programs (Medicare, Medicaid, SSI)

2. Disability Services / Rehabilitation Counseling

  • ADA accommodations
  • Accessing disability benefits and assistive technology
  • Independent living strategies

3. Case Management / Human Services

  • Assessments
  • Service planning
  • Documentation and care coordination

4. Supportive Housing / Housing Stability / HUD Programs

  • Fair Housing & Civil Rights laws
  • HUD regulations
  • Housing First & Rapid Rehousing models
  • Coordinated Entry systems

MINIMUM QUALIFICATIONS

Education/Background:

  • A Bachelor’s degree. Preference for majors in Social Work, Psychology, or Counseling is preferred; and two- three (2-3) years of experience in social service delivery with an elderly/family population, especially those experiencing housing instability, OR
    • A combination of a minimum of six years of work experience and education may be considered.
  • Bilingual, English and Spanish, preferred
  • Fingerprint Clearance Card
  • Successfully complete required pre-employment screenings
  • Employee will be expected to participate in continuous learning, competency building and maintenance of competency skills. Thirty-six (36) hours of training must be completed within the first year of employment. 

 Other Requirements

  • Commitment to maintain a role that empowers clients and intervene appropriately to meet service goals.
  • Interpersonal communication skills that allow for professional and positive relationships with coworkers, participants, families, and other community members, evidenced by non-judgmental approach, effective listening and positive interactions.
  • Sensitivity to socioeconomic and cultural characteristics of the populations served.
  • Creative thinking, adaptive personality, and a conviction about the capacity of people to grow and change. 
  • Ability to advocate, organize, problem-solve, and provide results for the target population.
  • Working knowledge of supportive services and other resources in the area served.
  • Ability to present operational information to personnel at all levels. 
  • Ability to travel statewide and work a flexible schedule if needed.  
  • Awareness and commitment to Housing First, rapid rehousing, coordinated entry procedures, and other housing-based strategies for ending homelessness.
  • Exceptional skill in customer service to facilitate positive on-going relationships between residents, on-site property management teams, and the community.
