TITLE: Facilitator
FLSA: Non-Exempt
SALARY: $28 hourly
SCHEDULE: Hybrid remote. You'll have opportunities to work-from-home while still prioritizing client services.
Wraparound offers child-focused, family-centered mental health services based on strength and needs. There are 10 principles to the Wraparound programs: Family Voice and Choice, Team-Based, Natural Supports, Collaboration, Community Based, Culturally Competent, Individualized Strength-based, Persistence, and Outcome Based. Our Wraparound teams work with our Parent Partners, Child and Family Specialist, and Facilitator to create goals. Together we create strategies to support families and track progress.
POSITION SUMMARY
The Facilitator is responsible for coordinating the activities of the child and family team and the development and implementation of individualized child and family team plans. The Facilitator participates in outreach and engagement to informal supports. The Facilitator collaborates and works with the WRAP Program Manager to discuss progress of cases and staffing challenges. The Facilitator is the point of contact with the DMH WRAP Liaison and partners to problem obstacles and/or barriers presented by clients during treatment.
The Facilitator works closely with the Program Manager, ensuring services are meeting agency, programmatic and DMH standards. The Facilitator oversees the coordination of case management services, providing unconditional support to children and families.
The Facilitator does a variety of activities, such as:
- Provide leadership in the assessment, development, implementation, and evaluation of individual child and family plans.
- Coordinate, schedule, and facilitate child and family team meetings for development of individualized child and family plans.
- Collaborate with outside stakeholders to coordinate with WRAP team that include, but are not limited to: DMH WRAP, LAUSD, LA Probation & Regional Center.
- Monitor and support the provision of unconditional care by assessing individual plans and making changes as needed, in collaboration with child and family team.
- Participate in outreach and engagement activities.
- Provide transportation for clients for linkage services, or other pre-approved outings, as needed.
- Participate in the on-call weekend rotation for crisis calls.
MINIMUM QUALIFICATIONS AND COMPETENCIES
Education /Background:
- Bachelor’s degree in social services or a related field highly preferred. Must meet one of the following requirements below:
- If candidate has an AA degree in social services or a related field, candidate must have 3 or more years’ experience in working directly with seriously disturbed individuals and families.
- If candidate has a BA/BS degree in social services or a related field, candidate must have 2 or more years’ experience working directly with seriously disturbed individuals and families.
- Bilingual Spanish speaking required.
- CA Driver License required to provide independent transportation for clients for linkage services, or other pre-approved outings, as needed.
Other Requirements:
- Experience working with multiethnic/racial clients and communities.
- Ability to coordinate meetings and engage potential clients being referred to program.
- Demonstrated ability to successfully navigate stressful situations during meetings.
- Excellent verbal and written communication, organizational and planning skills.
- Ability to maintain confidentiality of records and information.
- Ability to prepare effective written materials including correspondence and reports.
- Ability to be flexible and multitask, handling several tasks simultaneously.
- Ability to stay focused on duties and projects and get them completed on deadline as assigned.
- Proficient with Microsoft Office (Word, Excel, PowerPoint, Outlook, especially Microsoft teams).
HYBRID REMOTE
Position is eligible for hybrid remote. This role requires telehealth and in-person appointments in the field and in the office, meeting with clients, prospects and community partners as needed. Employee must be available to work during office hours. Employees in crisis programs must be available to participate in the on-call rotation schedule as assigned.
