Chicago Family Health Center (CFHC) is seeking a Community Health Worker – Wellness & Preventive Care Coordinator to support health equity and access across our communities. This position works under the guidance of the Manager of Care Coordination to promote wellness, increase preventive care, and reduce disparities in healthcare access. Key Responsibilities
Community Health & Patient Navigation:
- Serve as a liaison between CFHC, health/social service providers, and community members to improve access to care and enhance cultural competency in service delivery.
- Identify and communicate the purpose, benefits, and impact of CFHC programs to patients and targeted populations.
- Distribute educational materials (flyers, brochures, etc.) to inform the community of CFHC services and promote engagement in wellness and preventive care.
- Advise patients and community groups on health improvement strategies, including healthy diet, physical activity, and chronic disease prevention.
- Assist patients in developing personal health management goals and understanding their care plans.
- Support patients in referrals, completing applications, scheduling appointments, and coordinating transportation services.
- Educate patients on the appropriate use of emergency services and available alternatives.
- Maintain accurate and timely documentation of all activities, service plans, and patient follow-ups in the Electronic Health Record (EHR) and related systems.
- Establish and maintain positive, supportive relationships with patients, providers, and community partners.
- Gather and relay provider and patient feedback to support continuous improvement in patient navigation and preventive care strategies.
- Actively contribute to initiatives focused on breast cancer screening quality improvement and overall patient engagement in preventive services.

Requirements
Education & Certification:
- High School Diploma or GED required
- Community Health Worker (CHW) Training/Certification
- Additional training or experience in a healthcare setting (e.g., medical assistant, care coordinator, or patient navigator) preferred.
- Minimum of 1–3 years of experience in community outreach, health advocacy, or working with health and social service organizations.
- 1–2 years of experience in a clinical or healthcare environment is preferred.
- Experience working with diverse or vulnerable populations strongly preferred.
- Experience with data entry and generating reports is a plus.
- Strong understanding of healthcare systems, care coordination, and patient navigation processes.
- Proficiency in using Electronic Health Records (EHR) systems.
- Excellent verbal and written communication skills.
- Strong interpersonal and relationship-building skills with the ability to collaborate effectively in a team setting.
- Highly organized with strong time-management abilities and attention to detail.
- Ability to work independently, manage multiple priorities, and maintain professionalism under pressure.
- Ability to follow organizational protocols and maintain confidentiality at all times.
- Demonstrated cultural competence and sensitivity when working with diverse populations.
- Ability to appropriately handle confrontation and resolve conflict constructively.
- Reliable transportation required.
- Flexibility to work evenings and weekends as needed.
- Bilingual or multilingual skills preferred (requirement may vary by department).
- Punctual, reliable, and enthusiastic about learning and contributing to positive community health outcomes.
