Pay Range
$24.27 - $36.10 /hour

- Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.
- Collaborate with the interdisciplinary care team to coordinate safe, effective discharge plans and facilitate timely referrals to appropriate post-acute and community resources.
- Advocate for patients and families by connecting them with support services and assisting with applications for available community and financial resources.
- Educate patients, families, and healthcare team members on available services, care options, and community resources to support successful transitions of care.
- Document assessments, interventions, and care coordination activities while maintaining compliance with organizational, state, and federal standards.
- Certified Social Worker (CSW) in Nebraska
- Master's degree in Social Work (MSW) or Bachelor's degree in Social Work (BSW) with the intent on receiving a Master's
- Basic Life Support (BLS) from the American Heart Association
