St. Luke’s Health–Brazosport Hospital is located on a beautiful, 25-acre campus in Lake Jackson, Texas. Offering state-of-the-art diagnostic and comprehensive treatment services, our hospital is home to a 154-bed patient tower, level III trauma center, advanced cardiac care center, and full-service, multidisciplinary cancer center. Our team consists of over 100 board-certified physicians and nearly 600 highly skilled team members to meet our patients’ medical needs. Pay Range
$34.00 - $50.58 /hour

- Conduct and document psychosocial assessment of patients
- Share responsibility in identifying appropriate decision makers if patient is without capacity, educating all in patients’ rights and encouraging participation in decision‐making and care
- Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs
- Identifies mental health needs of patients and Provides counseling and referral as indicated
- Collaborates with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community by coordinating placement with home health agencies, long term acute hospitals (LTACs), skilled nursing facilities (SNF) and assisted living facilities, nursing home placement, acute rehab, psych placement, and custodial care facilitation
- Serves as a resource to the patient, family, and healthcare tem regarding emotional and behavioral issues and refers to community services available to adolescent, adults and elders
Job Requirements
- Masters Other from a Council on Social Work Education accredited graduate school of social work and Three (3) years clinical social work , upon hire or
- Three (3) years allied health practice, with hospital or medical background or experience
- Master Social Worker: TX, upon hire
