In 2014, Phoenix Children’s Hospital launched a 22-bed pediatric unit inside the hospital. Mercy Gilbert opened in 2006 and has been a vital part of the growing community. Today it is the fifth largest employer in Gilbert. During the fiscal year ending June 30, 2016, Mercy Gilbert provided more than $24 million in community benefit. Pay Range
$57.37 - $85.33 /hour

- Oversees the daily operations of the Care Coordination program, ensuring accountability, effectiveness, efficiency, and compliance with regulatory and accreditation agencies, including direct planning and implementation across the continuum of care.
- Accepts multiple management tasks to handle financial, operations, managed care & staffing issues within the Care Management departments.
- Serves as clinical advisor to care management across the continuum of care for all programs, including but not limited to, participation in program development, payer relations, facility relations; patient tracking across the continuum of care; decisions regarding alternatives to acute care which consider quality and financial factors.
- Analyzes data and develops strategies and action plans to address opportunities for improvement.
- Develops department and division strategies to ensure efficient operations and goal achievement.
- Maintains relationships with medical staff members, CNO, nurse VPs, Directors/managers, department managers and technical Directors to support intradepartmental goals and advice on activities pertaining to outcomes.
Job Requirements
- Minimum Bachelors of Nursing required
- Masters in either nursing, allied health or business administration preferred
- Seven years administrative and health care system experience and Seven years of clinical and care management experience
- Five years of leadership experience
- Registered Nurse: AZ
- Certified Case Manager and Accredited Case Manager upon hire
