A Day in the Life of a CRMG Diabetes Program CoordinatorThe Diabetes Program Coordinator works as part of the diabetes self-management education and support (DSMES) team and is primarily responsible for coordinating the development, implementation and monitoring of the accredited diabetes education program within the healthcare system. This position also assists in maintaining optimal care of Cheyenne Regional patients through appropriate training by providing direct patient care to patient/families with diabetes in the outpatient setting.Why Work at Cheyenne Regional?403(b) with 4% employer matchANCC Magnet Hospital21 PTO days per year (increases with tenure)Education Assistance ProgramEmployee Sponsored Wellness ProgramEmployee Assistance ProgramLoan Forgiveness Eligible Here is What You Will Be Doing:Responsible for maintaining adherence to the National Standards for Diabetes Self-Management Education and Support (DSMES) as outlined by the American Diabetes Association (ADA), at all times during the recognition period, to know when recognition will expire and to submit a timely application for renewal. Oversees the ADA program including planning, implementation, and evaluation. Serves as resource on diabetes-related issues for clinical staff, physicians, and community. Serves on applicable committees as needed and helps with marketing to promote service line. Provides direct patient care, including reviews blood glucose log and patient chart review; education plans; educate the patient in the areas(s) of assessed needs and documents education outcomes; follow-up on selected behavioral goal(s) achievements as identified; requests physician order for diabetes education when appropriate. Works collaboratively with physician and staff to optimize diabetes care. Actively participates in patient care communication with the patient’s ordering provider or other healthcare team members a summary of the education plan or education provided, and outcomes. Trains patients in the basics of disease process; nutrition recommendations for diabetes; self-monitoring of blood glucose; medication instruction (including insulin administration); role of physical activity; prevention of acute and long-term complications; appropriate follow-up care and community resources. Documents care and education of patient legibly and accurately in the patient chart, maintaining current and reliable patient training materials and education forms. Desired Skills:Good communication skills with patients, families, and staff Strong analytical ability with initiative and good judgement Ability to teach a variety of individuals patiently and tactfully Strong skills in education, including familiarity with adult and child learning theories, curriculum development and various teaching methods Ability to perform professional skills according to local, state, and national standards of practice in diabetes patient education Ability to interact respectfully with diverse cultural and socio-economic populations Here is What You Need:License (must meet one of the following): Current Wyoming Registered Nurse (RN) License from the Wyoming State Board of Nursing Current enhanced Nurse Licensure (eNLC) Current Wyoming Dietitian License from the Wyoming Dietetics Licensing BoardOne (1) or more years of clinical experience in an inpatient and/or outpatient setting14 Days: Cheyenne Regional AHA RQI within fourteen (14) calendar days of start date5 years: Current Certified Diabetes Care and Education Specialist (CDCES) Certification from the Certification Board for Diabetes Care and Education (CBDCE) within five (5) years of start dateNice to Have:Bachelor’s degree in nursing or dieteticsKnowledge of American Diabetes Association program accreditation requirementsSupervisory or management experienceAbout Cheyenne Regional:Cheyenne Regional Medical Center was founded in 1867 as a tent hospital by the Union Pacific Railroad to treat workers injured while building the transcontinental railroad.
