We are hiring an Adult Health Nurse to join our amazing team in Cheyenne, Wyoming.

As an Adult Health Nurse, you will:
• Perform medical case management services to assist clients to determine eligibility for the Wyoming Ryan White Program.
• Monitor, evaluate and assess the participant’s plan of care and modify the action plan or goals to accommodate the client’s changing needs at regular intervals as required to Program guidelines.
• Perform LT101 assessments with clients for the process of obtaining and/or maintaining Medicaid services.
• Become competent in public health emergency preparedness activities.
If you are the right person for this opportunity, you will have the following skills and experience:
• Associate’s degree in Nursing
• Must be licensed or eligible for licensure as a Registered Nurse (RN) in the State of Wyoming
• CPR Required
Wage: $29.00 to $36.25 based on experience
