Position Summary
We are seeking a dedicated Resident Care Coordinator (LPN/RN) to lead and oversee care for our 32 residents in both assisted living and memory care settings. This vital role combines leadership, clinical expertise, and compassion to ensure that each resident receives the highest quality of care.
The ideal candidate will be a strong leader with excellent communication, organizational, and problem-solving skills — and, most importantly, a genuine love for working with seniors and supporting their care teams.
Key Responsibilities
- Coordinate and oversee the daily care of residents within the community
- Lead, educate, and support direct care staff to ensure quality care delivery
- Develop, implement, and document individualized care plans
- Assess residents’ health needs and communicate closely with physicians and families
- Build and maintain strong relationships with outside providers, staff, and families
- Promote a culture of compassion, respect, and dignity in all interactions
- Level of Care Assessments on Residents
Qualifications
- Licensed LPN or RN in the State of Michigan (required)
- Previous experience in assisted living, long-term care, or memory care preferred
- Strong leadership and communication skills
- Excellent organization and documentation abilities
- Must be able to pass a criminal background check
