Intake Specialist and Home Visit Specialist

JOB DESCRIPTION

Home Visit Specialist for the area Seattle, will train, must be a driver, have own vehicle, assist with intake of new clients and other duties as assigned by Program Manager.

DETAILED DUTIES

• Plans and supervises home care services to contracted clients;
• Performs Case management functions including preparation of Care Plans and subsequent procedural actions;
• Performs Initial home Visits, and scheduled In-home visits in accordance with strict timelines;
• Ensures compliance with all the regulatory requirements;
• Communicates regularly with Case Managers, clients and caregivers as situation demands;
• Ensures timely response to and satisfactory resolution of Client and caregiver issues;
• Maintains continuous communication with the Program Manager;
• Maintains good communication channels with office staff, clients, caregivers, and case managers;
• Answers phones in a professional and pleasant manner, triage and problem solve in an effective manner;
• Performs any other duties as assigned by the Program Manager.

Job Type: Full-time

Pay: $24.00 - $25.00 per hour

Education:

• High school or equivalent (Preferred)

Experience:

• Computer skills: 1 year (Preferred)

License/Certification:

• Driver's License (Required)

Ability to Relocate:

• Seattle, WA 98104: Relocate before starting work (Required)

Willingness to travel:
