Position Summary
ChesPenn is seeking a dedicated and patient-centered Family Medicine Physician to provide comprehensive primary care to adults and children at our Chester, PA location. This role is ideal for a physician who thrives in a collaborative, community-focused environment and is passionate about serving medically underserved populations.
Qualifications
- Graduate of an accredited medical school
- Completion of an approved residency program in Family Medicine
- Unrestricted Pennsylvania medical license
- Board eligibility required; board certification preferred
- DEA certification
- Current COVID-19 and flu vaccinations required
- Experience in ambulatory primary care or community health preferred
Knowledge, Skills, and Abilities
- Strong written and verbal communication skills
- Excellent diagnostic and critical thinking abilities
- Positive, inclusive, and patient-focused demeanor
- Familiarity with trauma-informed care principles preferred
- Ability to work effectively as part of a multidisciplinary healthcare team
- Valid driver’s license and access to a licensed automobile preferred
- Ability to meet approved physical and medical standards
Scope of Responsibilities
Clinical:
- Provide routine and preventive care, including physical exams and screenings
- Manage acute and chronic conditions in an outpatient setting
- Conduct follow-up visits for patients referred to specialists
- Educate patients on health maintenance and ongoing care
- Collaborate with nurses, medical assistants, and administrative staff
- Supervise nurse practitioners and physician assistants
- Participate in emergency call coverage rotation, including weekends and holidays
- Maintain patient confidentiality and professional conduct at all times
- Perform additional duties as assigned by supervisor
Productivity:
- Maintain productivity levels in accordance with BPHC grant standards
- Meet targets for patient encounters and visits per patient annually
Record-Keeping & Administration:
- Document patient care accurately in the Electronic Medical Record (EMR) system
- Build relationships with community agencies and hospitals
- Lead or support medical and public health initiatives as needed
- Contribute to the development of the annual healthcare plan
- Engage in continuing medical education and professional development
