What You’ll Do
- Translate scientific language into clear, understandable information to help patients take charge of their health
- Assess patient needs and readiness to adopt healthier behaviors
- Develop, implement, and improve educational materials and tools that inspire positive change
- Conduct health education sessions with patients and families, and participate in community outreach
- Assist in training and mentoring Community Health Workers
- Collaborate closely with care teams to support patient health goals
- Lead tobacco cessation programs and travel between service locations as needed
Qualifications
- Bachelor’s degree in public health, Health Education, Nursing, or related field
- Minimum 2 years’ experience in community health education, outreach, or similar roles
- Valid driver’s license and current CPR certification
- Excellent interpersonal and communication skills, with leadership and independent learning abilities
