Summary
The Licensed Practical Nurse (LPN) is responsible for assisting in assessing, planning, implementing, and evaluating patient care while under the direction of a Registered Nurse or physician. This includes maintaining accountability for all delegated patient care given during a shift; working effectively with patients, families, and other members of the healthcare team; and practicing within the guidelines of established nursing care standards and policies and procedures.
Job Responsibilities
- Assist with patient education and document clinical information on the medical record
- Pre-assess patient by reviewing history and collecting current information: review of systems
- Obtain vitals for each patient as applicable
- Chaperone patient encounter with physician as requested and/or required
- Perform point of care testing and record results into the electronic medical record.
- Prepare requisitions for laboratory requests
- Coordinate homecare and physical therapy as ordered by the physician via telephone or fax
- Order prescriptions and/or coordinate pharmaceutical orders via telephone and/or electronic means as ordered by the physician
- Respond to telephone requests from patients and/or caregivers in a timely manner record in electronic medical record as applicable.
- Obtain patient laboratory, radiology, test results, and specialty reports, as required
- Prepare room and patient for provider encounter
- Provide intervention ordered by provider (medication, dressings, specimen collection, irrigations, immunizations, suture removal, and IVs)
- Initiate interventions and health education according to provider guidelines
- Educate and provide preparation materials for diagnostic studies (i.e. Lab, Radiology, EKG)
Education
Minimum Required Education: Graduation from an accredited Practical Nursing School Preferred Education: Not applicable
Certification:
For medical practices: 1-year recent related experience preferred
For employee wellness: 1-year of recent related experience required
Applicants must be licensed to work as a Licensed Practical Nurse in the Commonwealth of Virginia or in North Carolina and must obtain BLS certifications on hire.
