Job Overview

Chesapeake Home Health Care is seeking an experienced, organized, and dependable Registered Nurse (RN) Field Supervisor to join our clinical leadership team.

This is a field-based supervisory position responsible for overseeing approximately 35–45 clients receiving in-home care services throughout Maryland. The RN Supervisor will provide clinical oversight and supervision of Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), Personal Care Aides (PCAs), and Care Companions assigned to our clients.

The ideal candidate has strong home care experience, excellent assessment and documentation skills, and the ability to independently manage a field caseload while ensuring that clients receive safe, appropriate, and high-quality care.

This position requires regular travel to clients' homes and is best suited for an RN who enjoys working independently in the community while remaining an active member of the agency's clinical and administrative team.

Key Responsibilities

  • Manage and provide clinical oversight for approximately 35–45 active clients.
  • Conduct initial and ongoing nursing assessments and required reassessments.
  • Complete scheduled in-home supervisory visits for assigned clients and caregivers.
  • Supervise CNAs, HHAs, PCAs, Care Companions, and other applicable direct-care staff.
  • Observe field staff performance and ensure services are being provided according to the client's individualized plan of care.
  • Develop, review, and update client plans of care as appropriate.
  • Identify changes in client condition and communicate concerns to the Director of Nursing, provider, family, case manager, or other appropriate parties.
  • Ensure caregivers understand client-specific needs, responsibilities, precautions, and reporting requirements.
  • Evaluate caregiver skills and competencies and provide education, retraining, or corrective instruction when necessary.
  • Ensure appropriate nursing delegation and supervision consistent with Maryland requirements and the caregiver's scope of practice.
  • Review clinical and caregiver documentation for accuracy, completeness, timeliness, and compliance.
  • Maintain required supervisory, assessment, competency, and clinical documentation.
  • Assist with quality assurance activities, clinical record audits, and corrective action plans.
  • Maintain assigned client records in a survey-ready and audit-ready condition.
  • Communicate regularly with clients, families, caregivers, scheduling staff, and agency leadership.
  • Assist with caregiver orientation, education, competency validation, and ongoing training.
  • Report concerns involving suspected abuse, neglect, exploitation, unsafe care, or other significant client-safety issues in accordance with agency policy and applicable reporting requirements.
  • Assist the Director of Nursing and Administrator with maintaining compliance with applicable Maryland Residential Service Agency (RSA), Maryland Board of Nursing, Maryland Department of Health, Medicaid, HIPAA, and COMAR requirements.
  • Perform other clinical and supervisory duties consistent with the RN's scope of practice and agency needs.

Qualifications

Required:

  • Current and unrestricted Registered Nurse (RN) license in the State of Maryland.
  • Graduate of an accredited nursing program.
  • Current CPR/BLS certification.
  • Valid driver's license and reliable transportation.
  • Ability and willingness to travel to clients' homes throughout the assigned service area.
  • Strong nursing assessment, documentation, organizational, and communication skills.
  • Ability to work independently and effectively manage multiple clients, caregivers, and regulatory deadlines.
  • Comfortable utilizing electronic medical records (EMR) and electronic documentation systems.

Preferred:

  • Minimum of 2 years of nursing experience.
  • Previous home care, home health, community health, long-term care, case management, or supervisory nursing experience.
  • Experience supervising CNAs, HHAs, PCAs, Care Companions, or other direct-care staff.
  • Familiarity with Maryland Residential Service Agency requirements and Maryland Board of Nursing standards.
  • Experience conducting home assessments, supervisory visits, caregiver competencies, and clinical documentation audits.

Position Requirements

This is a field-based position requiring regular travel between client homes. The RN Supervisor will be expected to maintain an organized schedule to ensure assessments, supervisory visits, documentation, follow-up activities, and other clinical responsibilities are completed within required timeframes.

The position requires the ability to work independently while maintaining consistent communication with the Director of Nursing, Administrator, scheduling department, and other members of the Chesapeake Home Health Care team.
