POSITION SUMMARY
Transport scheduled, and at times unscheduled, patients and parents to CHC offices for appointments. This position reports to the Director of Population Health.

PRIMARY TASKS & DUTIES
- Provide safe and reliable pick up and drop off of patients to the appropriate medical facility
- Keep accurate transportation logs and observe/report basic functions/malfunctions of vehicles to Transportation Outreach Partner or Manager.
- Physically assist passengers in evacuation of the vehicle in event of emergency.
- Immediately report any accident.
- Keep the interior of the assigned vehicle clean and presentable.
- Present a neat and professional appearance at all times.
- Complete and submit all paperwork in a timely manner.
- Maintain cooperative attitude with co-workers, management and passengers.
- Demonstrate excellent customer service skills.
- Demonstrate the ability to multi-task.
- Perform all other duties as assigned.
- Assist with scheduling and dispatching.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES
- Function in a multi-tasking, multiple priorities environment, while maintaining accuracy.
- Demonstrate excellent customer service skills
- Communicate with a diverse patient/customer population. Will be required to successfully manage patients and families in often stressful or conflict situations.
- Utilize a wide variety of office technology and tools to collect, enter, process data.
- Maintain valid driver’s license
- Complete and pass all training.
POSITION REQUIREMENTS
Education
· High School diploma or equivalent.
· Must be able to speak read, write and understand English.
· Must have a valid driver’s license.
· DOT will be required
· Computer knowledge is preferred.
