Key Responsibilities
- Perform initial and ongoing assessments of mobility, coordination, strength, and function.
- Administer, evaluate, and interpret tests to establish functional goals and outcomes.
- Provide written evaluations and plans of care, outlining occupational therapy needs, goals, progress, and expected results.
- Deliver occupational therapy services per physician orders and document accordingly.
- Maintain and modify the plan of care, including discharge planning and patient education on home exercises.
- Coordinate services with healthcare team members, including physicians, nurses, and caregivers.
- Supervise relevant personnel (e.g., OTA, HHA) as applicable.
- Complete assessments such as OASIS where permitted by regulations.
- Degree in Occupational Therapy from an accredited program.
- Current, unrestricted state license as an Occupational Therapist.
- Proficiency in clinical assessments, documentation, and compliance.
- Valid CPR certification, driver's license, and automobile insurance.
- Effective communication with staff, healthcare team, and families.
- Minimal supervision required for documentation and clinical duties.
