Clinical Social Worker, Diabetes

We are seeking a full-time social worker to provide clinical services in a multidisciplinary program focused on early-stage Type 1 Diabetes management.

Responsibilities Include

• Conducting psychosocial assessments, crisis intervention, and individual or family counseling, both in person and via telehealth.
• Collaborating with Patient Navigators to help patients and families access services and resources.
• Consulting with a multidisciplinary team on psychosocial issues such as coping, treatment challenges, family distress, trauma, child protection, and domestic violence.
• Developing new initiatives to support patients and families within the program.
• Supporting research and quality improvement efforts.
  
  

Qualifications Include

• Master’s degree in social work and Massachusetts licensure (LCSW required, LICSW preferred).
• Experience in medical settings and with child protection issues is beneficial.
• Ability to work in a fast-paced environment with strong teamwork and communication skills.
