Job Duties:

Provide personal care to hospice patients, including bathing, grooming, dressing, toileting, and mobility assistance

Support patients with activities of daily living (ADLs) while promoting dignity, comfort, and safety

Assist with basic homemaking tasks such as light housekeeping, meal preparation, and linen changes

Offer emotional support and companionship to patients and families during end‑of‑life care

Observe and report changes in the patient’s condition, behavior, or needs to the supervising nurse

Maintain accurate, timely documentation of care provided in accordance with agency policies

Follow individualized care plans and collaborate with the interdisciplinary hospice team

Assist with safe transfers, positioning, and use of mobility devices

Ensure a clean, comfortable, and supportive environment for patients

Adhere to infection control practices, safety protocols, and regulatory requirements

Respect patient rights, privacy, and cultural or spiritual preferences

Participate in team meetings, in‑service training, and ongoing professional development
Training/Licensure:
- Graduate from an approved training and competency course. Must be Certified Home Health Aide.
Knowledge/Skills/Ability:
- Must meet federal Home Health Aide training and competency requirements.
- Ability to work independently, make accurate, and at times, quick judgments.
- Ability to respond appropriately to crisis outside of a hospital setting.
- Acceptance of and adaptability to different social, racial, cultural and religious modes.
Experience:
- Minimum 2 years of experience as a nursing assistant, preferred.
- Active patient contact within past three years, preferred.
