We are seeking a compassionate and reliable Home Health Aide (HHA) to provide personal care and supportive services to patients in their homes. In this role, you will assist with daily living activities, help maintain a safe and comfortable environment, and support patients in achieving their highest level of independence. Key Duties and Responsibilities

Provide personal care services including bathing, grooming, dressing, toileting, and mobility assistance

Support patients with activities of daily living (ADLs) while promoting dignity, comfort, and safety

Monitor and report changes in the patient’s condition, behavior, or needs to the supervising nurse

Follow individualized care plans and document care provided in accordance with agency policies

Assist with safe transfers, positioning, and use of mobility devices

Offer companionship, emotional support, and encouragement to patients and families

Adhere to infection control practices, safety protocols, and regulatory requirements

Maintain confidentiality and respect patient rights, cultural values, and personal preferences

Participate in ongoing training, team meetings, and professional development activities
Qualifications

Active Home Health Aide certification or completion of state‑approved training (as required by state regulations)

Current CPR/First Aid certification. Will provide training if needed.

Experience providing personal care or in‑home support preferred

Ability to follow care plans and communicate effectively with nurses, patients, and families

Physical ability to assist with lifting, transfers, and mobility support
