Responsibilities
- Conduct thorough patient assessments to determine needs, develop individualized care plans, and monitor ongoing conditions.
- Administer medications and treatments in accordance with physician orders and hospice protocols, managing symptoms effectively.
- Collaborate with interdisciplinary teams—including physicians, social workers, chaplains, and therapists—to coordinate holistic patient care.
- Educate patients and families on disease progression, medication management, and comfort measures to promote understanding and peace of mind.
- Manage documentation accurately within medical records systems while adhering to HIPAA regulations to protect patient confidentiality.
- Perform vital signs monitoring, physical examinations, and diagnostic evaluations to inform ongoing care strategies.
- Assist with discharge planning and coordinate post-acute or outpatient services as needed for seamless patient transitions.
Qualifications
- Current RN licensure in good standing with the appropriate state licensing board.
- Critical care experience preferred, including ICU or emergency medicine backgrounds; experience in Level I or Level II trauma centers is highly valued.
- Strong knowledge of physiology, medical terminology, and acute care practices essential for effective symptom management.
- Demonstrated experience with hospital medicine, infectious disease care, behavioral health, geriatrics, or post-acute settings is advantageous.
- Familiarity with medical records systems, discharge planning processes, utilization management, and case management principles.
- Excellent patient assessment skills coupled with the ability to perform diagnostic evaluations confidently.
- Ability to work independently while collaborating effectively within multidisciplinary teams in inpatient or outpatient environments.
