Cherish Hospice is seeking a part time PRN Registered Nurse (RN) in the Canton area..

Responsibilities Of An PRN Hospice RN-

• Collaborate with the care team including Aide, Social Worker, Chaplain
• Assessing clients' physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans
• Working with clients, family and friend support networks and health care professionals to put care plans in place
• Listening to clients' concerns and providing counseling or intervention as required
• Recording clients' progress including charting referrals, home visits and other notable interactions
• Travel as necessary to service patients in their residence of care, which includes home, assisted living facilities, long term care facilities, and hospitals.
  
  
  

Qualifications for a Registered Nurse (RN) include, but are not limited to:

• RN License (Required)
• Driver's License (Required)
• BLS Certification (Preferred)
• Hospice or Home Health: 1 year (Preferred)
• Hospice and Palliative Medicine (Preferred)
