Cherish Hospice is seeking a PRN Dietitian.
PRN Dietitian Responsibilities:
- Provide direct dietary assessment of patient.
- Provide education to patient and caregivers regarding diet management.
- Appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
- Provide counseling and education to IDG, nursing staff and physician, as appropriate.
- Bachelor’s degree in nutrition or related subjects, preferred.
- Current State Driver’s License.
- Two years of experience as a Dietician in a clinical care setting, hospice or home health preferred.
- Ability to establish a plan of care to meet the dietary needs of the patient.
