We are seeking a Geriatric Assessment Practitioner to join our clinical operations team.
This individual will be responsible for assessing and managing the health needs of elderly patients under the guidance of the Regional Chief Clinical Officer.
Responsibilities:
- Assessing and managing the health needs of elderly patients, including geriatric assessment, medical history, physical exam, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to ChenMed standards for quality, service, productivity and teamwork
- Developing and implementing a patient management plan in response to the data obtained from assessments and analysis of trends in patient conditions
- Communicating effectively with patients, families, and other healthcare professionals to ensure comprehensive and coordinated care
- Maintaining accurate and up-to-date patient records and contributing to quality improvement initiatives
