The Chemung County Health Department’s Certified Home Care Agency is seeking a Registered Nurse (RN), to work as a Case Manager, evaluating and treat patients in a home care setting

Possession of a license by the State of New York to practice as a Registered Professional Nurse is required for this position.
Key Functions
- Evaluate and treat patient in their own home within the Chemung County community
- Provide patient, caregiver and family support and education
- Development and evaluation of plan of care in partnership with the patient and
caregiver
- Preparing of clinical notes
- Performing clinical duties such as: IV therapies, wound care, and disease process
teaching
- Collaborate and consult with the Home Care team, or appropriate welfare or health agencies to ensure patient receives proper services and follow up care
Job Type: Full-time
Pay: $32.31 - $33.79 per hour
