Responsibilities
- Conduct Needs Assessments and develop plans for resources to meet those needs.
- Develop individualized care plans that incorporate discharge planning, utilization management, and post-care coordination.
- Collaborate with multidisciplinary teams including social workers, behavioral health specialists, addiction counselors, and medical staff to optimize patient care strategies.
- Facilitate diagnostic evaluations and coordinate medical records review to support case management decisions.
- Provide patient service by advocating for patient needs, supporting mental health counseling efforts, and structuring their discharge process.
Qualifications
- Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams.
- Ability to navigate HIPAA compliance standards while managing sensitive patient information.
- Bachelors Degree in a Human services-related field ONLY.
Pay: $22.00 per hour
Expected hours: 30.0 – 40.0 per week
