Hospice Nurse Practitioner

Position Summary

The Hospice Nurse Practitioner works in collaboration with the hospice staff to provide comprehensive, quality hospice care services to patients under the direction of the Hospice Medical Director. The Hospice Nurse Practitioner on an as needed basis will fulfill the requirements of the "Face to Face Encounter" with a Medicare beneficiary enrolled in Hospice. In addition, the Hospice Nurse Practitioner will be available as needed to provide patients visits in collaboration with the assigned hospice nurse in order to assist with pain and symptom management as well as the evaluation of terminal prognosis.

Hours: Per Diem

Pay Rate/Range: $100/hour for patient visits, $75/hour for administrative time

Qualifications

  • Must be a graduate of an approved, accredited school of professional nursing;
  • MSN required;
  • Currently licensed as a RN and Nurse Practitioner in good standing in Massachusetts;
  • 2-4 years of experience in a medical specialty involved in the care and treatment of patients diagnosed with a serious, advanced or terminal illness
  • Minimum of 2 years experience working in a hospice setting (preferred);
  • Excellent oral and written communications, interpersonal and clinical skills;
  • Demonstrates knowledge of federal, state and accrediting hospice agency requirements.

Physical Requirements

  • Have the ability to move intermittently throughout the work day, inclusive of sitting, standing, bending, pushing, pulling or moving objects and patients.
  • Heavy work; exerting 50-100 pounds of force occasionally and/or 25-50 pounds of force frequently to move objects including the human body.
  • Standing/walking; frequently and intermittently to accomplish tasks. May also chose to stand while completing administrative duties.
  • Lifting; occasionally raises objects including the human body from a lower to a higher position or horizontally from position to position.
  • Pushing/Pulling; occasionally uses upper extremities to push/pull objects including the human body in a sustained motion.
  • Climbing; frequently climb in and out of a car. May frequently ascend and descend stairs, uneven surfaces, or obstacles that may be located indoors or outdoors, and may not be handicapped accessible.
  • Stooping/Kneeling/Crouching; may occasionally stoop, kneel, or crouch during the provision of care.
  • Balancing; must maintain balance that is necessary for ordinary locomotion and body equilibrium.
  • Sitting; may sit frequently as when traveling to patient's home, providing care or while completing administrative duties.
  • Reaching; may have to reach to administer care, treatments and/or procedures.
  • Grasping; may frequently grasp and/ or handle objects, including medical equipment/supplies, the human body and a writing instrument during the provision of care.
  • Feeling; able to touch objects including the human body to perceive attributes such as size, shape, temperature or texture.
  • Talking; must frequently exchange information in person and on the telephone to patients, doctors, family members, staff members, by means of the spoken word.
  • Repetitive Motions; must frequently use upper and lower extremities in a repetitive motion, including writing and driving.
  • Sensory needs; must possess fine motor skills, visual acuity and those senses necessary for patient assessment/evaluation, communication and driving, including sight, touch, hearing and smell.
  • Reaching; must be able to reach periodically to accomplish tasks, either while performing social work tasks or administrative tasks.
  • Stooping/Kneeling/Crouching; may need to stoop, kneel or crouch during the work hours to accomplish tasks.

Performance Responsibilities

  • Represents Legacy Lifecare VNA and Hospice in a professional and responsible manner.
  • Adheres to the practice of confidentiality regarding patients, families, staff, and Legacy Lifecare VNA and Hospice.
  • Functions as an integral part of an interdisciplinary team in order to enhance the mission, policy, and philosophy of Legacy Lifecare VNA and Hospice..
  • Complies with all applicable laws, and regulations with respect to collaborative agreements.
  • Practices in a manner that is a model for professional and agency ethics, values and integrity, and that complies with the letter and spirit of legal aspects.
  • Provides services related to health maintenance, promotion, and end-of-life care.
  • May recommend medications, including controlled substances, working hand-in-hand with patient's primary care physician, and Hospice Medical Director.
  • Assists with recommending treatment and durable medical equipment as if necessary.
  • Consults with referring, and primary care physicians, and designees as needed, providing written reports on patient contacts, and communications.
  • Prepares, and maintains accurate patient records to support appropriate clinical care and coding for reimbursement for services provided in a timely manner when/if necessary or appropriate.
  • Ensures the clinical record, with all the information contained therein, is safeguarded against loss or unauthorized use when in his/her possession.
  • Attends, weekly IDG meetings as needed and/or requested by Medical Director
  • Attends all other appropriate meetings as requested, assigned or delegated by Hospice Medical Director and/or Executive Director.
  • Assists in providing training, and continuing education for staff when appropriate.
  • Meets with Executive Director weekly for support or as needed.
  • Completes Assessment for the face to face 180-day recertification of existing Hospice patients.
  • Works with the Hospice team to support an open access to Hospice in a timely manner.
  • Individually responsible to conduct him/herself in a safe manner during the performance of all job related duties. Also responsible to bring unsafe conditions to the attention of the person within the organization responsible for safety issues.

Working Conditions

  • Subject to environmental conditions: activities can occur inside and outside of the home.
  • Subject to a variety of physical conditions within the individual patient homes: safety issues may include basic home safety, fire safety, environmental safety and bathroom safety.
  • May occasionally be required to handle hazardous substances including blood, infectious materials and hazardous waste.

The salary range above represents the low and high end of the salary range for this position. The final salary offered will be determined based on a comprehensive evaluation of factors, which may include, but are not limited to, the candidate's experience, education, training, skills, qualifications, market and business considerations, and geographic location.
