Position: School Social WorkerLocation: Parker Middle School (Grades 5/6)Job Description: The Social Worker will provide ongoing support and intervention to students in grades 5 / 6 through a multi-tiered system of support model to strengthen student wellbeing and school success. This candidate will work collaboratively within the Parker School community to respond to student mental health and support academic growth.
The School Social Worker participates as an active member of the Parker School Counseling team to contribute to the schools development of a healthy, safe and supportive learning environment for grade 5 / 6 students. The Social Worker will be an integral member of the Parker School STRIVE Social-Emotional classroom to promote Social-Emotional Learning and behavioral health wellness. The successful candidate will demonstrate a strong commitment to establishing family partnerships to remove barriers using an inclusive and collaborative approach.Essential Responsibilities/Skills: * Experience and passion for working with 5th and 6th grade students.* Ability and initiative to work independently and collaboratively with a multi-disciplinary team including school educators, specialists, administrators, family and community partners.* Strong counseling/clinical skills and a passion for continued learning and professional development.* Maintain a caseload of assigned students to support behavioral health wellness and academic success.* Provide individual and group counseling to assigned student with measurable goals and progress monitoring.* Provide SEL Instruction and direct counseling support to students supported by the Parker School Social Emotional Classroom (STRIVE).* Initiate and participate in all school meetings related to student progress.* Ability to facilitate meetings and have ongoing communication with families and outside community partners.* Assist with the safety of students by facilitating risk assessments and providing crisis intervention and triage support.* Frequently assess student mental health needs, implement responsive plans and measure student growth.* Facilitate meetings as a member of the Parker School Student Support Team (SST).* Appropriately assess and report for suspected abuse/neglect as a mandated reporter. * Support school-wide SEL/MTSS initiatives to promote a positive school climate and a sense of belonging for all students.* Build and maintain relationships with parents/families of students served, including phone calls, meetings, emails and when needed home visits.* Highly collaborative approach and commitment to working as an integral member of the Parker School team by collaborating with administration, educators, members of the student services team, families and community partners.* Provide referrals for outside services and connect families to community resources.* Participate as a member of the Special Education team and provide counseling services to students on IEP's.* Other duties as assigned by school administrators.
