About the Role
We are seeking a relationship-driven Community Partnership & Growth Manager based in Indianapolis
/ Central Indiana to expand our presence and deepen our partnerships throughout the state. This is a
primarily field-based hybrid role that combines strategic planning with in-market relationship building,
networking, and business development across Indiana.

Key Responsibilities
New Partnership Development
• Build and strengthen relationships with Medicaid Waiver providers, case management teams, and
home- and community-based service organizations across Indiana, including:
◦ Area Agencies on Aging (AAAs), including organizations such as CICOA
◦ Medicaid Waiver providers
◦ Care managers and case managers
◦ Hospitals and healthcare systems
◦ Hospice and home health organizations
◦ Skilled nursing and long-term care providers
◦ Senior-focused nonprofit organizations
◦ Payer / insurance and community health organizations
• Identify and develop new referral and partnership channels
• Attend conferences, networking events, industry meetings, and community events across Indiana
• Conduct proactive outreach, prospecting, and relationship-building activity throughout your territory
Account Growth & Relationship Management
• Maintain and strengthen relationships with existing partner organizations
• Serve as a trusted point of contact for referral partners and community organizations
• Identify opportunities for expanded service utilization
• Support renewal, retention, and partnership growth efforts
Market Development
• Track market trends and partnership opportunities across Indiana senior care ecosystems
• Provide field insights to leadership around market needs and growth opportunities
• Collaborate with leadership on growth strategy and territory priorities
Required Qualifications
• 5 years of business development, partnership development, healthcare sales, or community
outreach experience
• Proven track record of proactive relationship building and new business growth
• Excellent written and verbal communication skills
• Strong presentation and networking skills
• Strong follow-through and organizational skills
• Ability to work independently and proactively manage a territory
• Based in Indianapolis / Central Indiana
• Ability to travel up to 50% throughout Indiana
• Valid driver’s license and reliable transportationPreferred Experience
Preferred Experience
We'd love to meet someone who has built trusted relationships within Indiana's healthcare, senior
care, and community support ecosystem. This may include professionals with experience in:
• Hospital and healthcare system partnership development
• Healthcare sales and provider relationship management
• Nursing leadership, clinical liaison, or care coordination roles
• Hospice, home health, or skilled nursing outreach
• Medicaid waiver or managed care provider relations
• Long-term care, senior services, or aging support organizations
• Case management, care coordination, or community referral networks
• Nonprofit organizations serving seniors and vulnerable populations
• Insurance, payer, or population health partnership roles
• Registered dietitian, nutrition services, or foodservice sales roles, particularly within healthcare,
senior care, or community meal programs
Experience working within Indiana's Medicaid Waiver, HCBS, or senior care ecosystem is strongly
preferred. If you have worked alongside providers, care managers, referral partners, or community
organizations that support seniors and their families, we especially want to hear from you.

Compensation & Benefits
• Total compensation opportunity: up to $110,000 annually
(base salary quarterly performance bonus)
• Base salary: $60,000
