The RN collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses. .

This Position Responsibilities Will Include
- Complete assessment of client and needs
- Education to client and families
- Chronic Care Management
- Develop care plan and establish goals
- Post-surgical and wound care as needed
- Wellness support
- Current, unrestricted California registered nurse license
- 3 years of experience as a registered nurse
- Home Health experience preferred
- Proof of Negative TB result
- Physical Screening Documentation within the last year
- Active California Drivers license and car insurance
- Cell phone with Internet Access
