In this role, you will partner with a multidisciplinary team to design and carry out customized care plans while maintaining clear, ongoing communication with clients, families, caregivers, and nursing staff. Strong problem-solving abilities and seamless collaboration with our administrative team are essential.
As the care manager, you will oversee and streamline service delivery across a network of caregivers, nurses, therapists, and external partners. This position calls for strong clinical judgment and proven experience navigating private-pay client dynamics and family relationships.
Option to work one weekend day and have one weekday off.

- Minimum 2–3 years of home health nursing experience required.
