Responsibilities
- Provide individual and group therapy sessions to patients in skilled nursing facilities
- Develop and implement personalized treatment plans in collaboration with other healthcare professionals
- Conduct assessments, screenings, and evaluations to identify mental health needs and develop treatment plans
- Collaborate with interdisciplinary teams to ensure comprehensive care for patients
- Document patient progress and treatment plans in accordance with industry standards
- Participate in regular consultation and case management meetings with other CHE Behavioral clinicians
- Must hold a current license as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT)
- Minimum of 3 years experience in mental health services
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Familiarity with electronic health records and documentation software
