- Provide individual, group, and family therapy to patients in long-term care and rehabilitation centers
- Develop and implement treatment plans in collaboration with patients, families, and healthcare professionals
- Conduct initial assessments and ongoing evaluations to monitor patient progress
- Participate in multidisciplinary team meetings to discuss patient care and treatment plans
- Maintain accurate and thorough records of patient interactions and progress
- Collaborate with other healthcare professionals to ensure comprehensive care

- Master's degree in Social Work (MSW) from an accredited program
