What are the responsibilities and job description for the Outreach Worker position at CHD Brand?
Center for Human Development (CHD) is currently seeking an Outreach Worker working within our Shared Living program. CHD’s Shared Living program provides an alternative residential setting to the 24-hour group home.
In the Outreach Worker role, you will:
Provide direct services to clients with developmental disabilities and autism who live independently in the community. Responsibilities include supporting clients' activities of daily living, advocating for individuals within various community settings, and providing opportunities to optimize their independence, rehabilitation and recovery. Develop professional and supportive relationships with clients and provide informal counseling and guidance with a person-centered focus.
Schedule: Full time – 30hours/week, schedule as followed:
Thursday and Friday-10:30am-6:30pm
Saturday and Sunday-11:30am-4:30pm
Pay rate: $20.00/hr.
Requirements:
- High School Diploma.
- At least three years of experience working in the field of human services with DDS, Clients, Autism or Mental Health Individuals.
- All formal and site-specific trainings with be provided by the agency and Shared Living Provider.
