CHD is seeking a qualified and compassionate Family Nurse Practitioner (FNP) to join our Primary Care team in a full-time, onsite role. The FNP will deliver comprehensive, patient centered primary care services to individuals and families across the lifespan, with a strong focus on preventive care, chronic disease management, and health promotion.
Key Responsibilities
• Provide comprehensive primary care services, including health assessments, diagnosis, treatment, and follow-up care
• Manage acute and chronic conditions in a primary care setting
• Perform physical exams, order and interpret diagnostic tests, and develop evidence-based treatment plans
• Prescribe medications and therapies in compliance with Massachusetts state regulations and clinical guidelines
• Deliver preventive care, health screenings, and patient education
• Collaborate with physicians and interdisciplinary care team members to ensure coordinated, high-quality care
• Maintain accurate, timely documentation in the electronic health record (EHR)
• Adhere to all clinical protocols, quality standards, and regulatory requirements
Required Qualifications
• Master’s or Doctoral degree from an accredited Nurse Practitioner program
• Current Family Nurse Practitioner (FNP) certification
• Active Massachusetts RN and APRN license (or eligibility for licensure in MA)
• DEA registration (or ability to obtain)
• Primary Care experience preferred; new graduates with strong clinical training may be considered
• Excellent clinical judgment, communication, and interpersonal skills
• Ability to work independently in a fast-paced primary care environment
Schedule & Work Environment
• Full-time, 40 hours per week
• Onsite Primary Care practice
• Monday–Friday schedule with occasional evenings/weekends, and or on call coverage
Compensation & Benefits
- Competitive salary commensurate with experience $135,000-$165,000 based on experience
