Per diem RN On Call

Help out our On Call Team! Weekday/overnight shifts are 4:30pm-8:30am. Shifts also available on Holidays and Weekends.

Responsible for working with RN Case Managers to ensure that care is available and provided on a 24hr/7day per week basis, which addresses the physical, emotional, psychosocial, and spiritual needs of patients and families who are experiencing life-limiting, progressive illnesses.

• Utilize the nursing process to assure a comprehensive plan of care;
• Assure ongoing assessment and revisions to a patient’s plan of care;
• Teach patient, family and/or staff on caring for patient;
• Assess patient, family and/or staff comprehension and ability to provide appropriate care;
• Identify significant clinical findings, draw conclusions and intervene appropriately;
• Promote communication through all levels of patient care including completing call log and on-call report;
• Participate in and attend clinical meetings;
• Give and receive “report” (status changes and needs of patients and/or families) to appropriate Case Managers at the beginning and end of each shift;
• Contact answering service at the beginning of each shift to confirm contact information (telephone number(s) where one can be reached;
• Contact administrative backup nurse appropriately for direction on administrative questions;
• Conduct or refer to administrative backup nurse admission/ informational contacts with patients and their families;
• Support and maintain regulatory compliance;
• Maintain timely and appropriate records/documents per Hospice policies and procedures;
• Maintain/enhance professional skills and requirements including education and training;
• Perform other tasks and duties as assigned;
• Take initiative as needed or when the opportunity arises;
• Provide on-call services and patient visits, as scheduled;
• Travel as required.
