Chatham 360° is a deferral and education program that develops and conducts a program of substance abuse prevention and education to individuals who are involved in the justice system. Our program is professional, caring, understanding, and dedicated to providing high quality substance abuse support to residents of Chatham County. This position is responsible for case management, education, referrals, and screening activities that provide support to individuals referred to Chatham 360 for our various programs. Early intervention and education to eligible defendants into appropriate treatment/referral programs is a primary objective. The responsibilities of the Substance Abuse Coordinator involve planning, organizing, screening, coordinating, educating, and monitoring client activities in accordance with Chatham 360’s policies and procedures. Decisions made are in response to changing conditions and often require discretion and conceptual analysis of data and situations which may frequently require the approval of the supervisor before implementation.
Essential Functions
- Evaluates a person’s eligibility for criminal justice programs, conducts risk assessments, and presents options to the judge, court, prosecutors, defense attorneys, and probation/parole officers;
- Initiates screening of individuals to plan for pretrial or early release;
- Assists in monitoring participants in pretrial services and other court diversion programs;
- Makes referrals for mental health and/or substance abuse treatment as indicated by professional assessment, diagnosis, or court order in coordination with supervisor;
- Works with community-based mental health and substance abuse providers to implement community treatment and individual service plans;
- Monitors individuals to reduce the number of failure to appears;
- Develops youth/young adult-based prevention/education programs
- Reports to the court as required on client compliance;
- Provides on-going individual case management and monitoring;
- Acts as liaison and consultant with a variety of community agencies and social service providers to maintain coordination of the intervention process for the individual involved in jail/court diversion programs, including but not limited to corrections, the courts, law enforcement, social service, and treatment providers.
- Records and tracks program data and defines how outcome data is collected, measured, and quantified;
- Provides assistance linking participants to appropriate resources for housing, employment, and other basic needs;
- Provides information regarding community resources and natural supports to individuals, providers, and the criminal justice system partners;
- Coordinates treatment interventions and referrals to lesser restrictive placements and outpatient treatment when possible;
- Performs all work duties and activities in accordance with County policies, procedures, and safety practices and
- Performs other duties as apparent or assigned.
Minimum Qualifications
Bachelor’s Degree from an accredited college or university majoring in Public Administration, Public Policy, Political Science, Economics, Business Administration, Social Work or related field and five (5) years of progressive responsible jail alternative program experience.
OR
Bachelor's Degree in a human services field and a minimum of two years of case management experience working with a criminal justice population or a substance use/mental health population; or equivalent combination of experience and education with treatment and diversionary programs or courts in the criminal justice system preferred.
Special Requirements
Valid driver's license in the State of North Carolina.
Substance abuse background training preferred.
Ability to work a flexible schedule, including on-call and crisis management.
Must be registered with North Carolina Substance Abuse Professional Practice Board.
Must obtain certification/license within 5 years after registering North Carolina Substance Abuse Professionals Practice Board (NCSAPPB).

Salary grade 59; $38,382
