Chase Center Rehabilitation and Nursing Services is seeking a dedicated and skilled Physical Therapist (PT) to join our in-house rehabilitation team. Responsibilities
Perform comprehensive physical therapy evaluations
Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
Deliver evidence-based therapeutic interventions
Monitor resident progress, update goals, and adjust treatment plans as needed
Accurately document evaluations, treatments, and outcomes in compliance with all regulatory requirements
Collaborate with the interdisciplinary care team to optimize resident outcomes
Educate residents, families, and caregivers on therapy goals, progress, and home exercise programs
Qualifications
Current Physical Therapist (PT) license in the state of Indiana
Graduate of an accredited Physical Therapy program
Must be at least 18 years of age
Skilled nursing or long-term care experience preferred
