JOB DESCRIPTION: Long-Term Care Experience a must!
- Analyzing patients’ health status, goals and dietary restrictions
- Creating individualized nutrition plans and meal plans
- Educating and counseling patients and families on diet and nutrition
- Tracking and assessing patients’ dietary intake and progress
- Monitoring residents weight trends
- Working with other healthcare professionals to coordinate care
- Participation in inderdisciplinary meetings with clinical staff and department heads
- Valid Registered Dietician for state of MA
