What are the responsibilities and job description for the Residential Coordinator position at CHARTIERS CENTER?
Chartiers Center is a private, non-profit corporation funded by state, county, 3rd party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day and recreational services for adults with intellectual disabilities, homeless outreach and housing program.
As a Mental Health Residential Coordinator with Chartiers Center in our Residential Treatment Program you are responsible for providing assistance to adults facing mental health challenges.
Job duties include:

Develop, implement, monitor, and document Residential Service Plans for an assigned caseload of residents.

Completing necessary skill training in the areas of daily living through hand over hand assistance, direct verbal and written presentation of information and observation, as assigned.

Accurately complete all assigned observation, intervention, documentation of resident self-administration of medication, medication counts and necessary follow-up in accordance with Program policies and procedures.

Accurately implement recommendations of the treatment team to ensure consistency within the program.

Maintain resident records in accordance to policies, procedures and regulations.

Complete laundry, cleaning and shopping as required.

Effectively respond to, report and document all emergencies/crises/incidents in accordance to relevant policies, procedures and regulations.

Assist the Director of the Residential Treatment Program to effectively oversee the daily operations of the program and in scheduling of staff /overall management of the schedule.

Minimum Qualifications-
-
Bachelor's degree in Human Service field and three (3) years’ experience in a behavioral health setting.

Valid PA Driver’s License.
